I have a hard time believing that it is the result of something like alternate realities, but I really don't think the unreliability of human memory can account for something on the level of the Berenstein Bears thing. So many people, including every person I've ever asked, remember it as Berenstein. If I had not been shown evidence to the contrary, I'd be 100% sure it was Berenstein.
I don't think we know enough to say we're precisely sure why something like this happens, whether it's claiming alternate realities or simply unreliable human memory.
